Physician Appointments
We track symptoms and developments and monitor reactions to medication changes. We accompany you to your appointments to communicate concerns and changes to your doctors. We keep an updated medication list and ensure your primary care physician and all specialists know what changes are being made.

Urgent Care & ER Visits
24/7, day and night, we're available to you. Our team will meet you at the emergency room with all important documents, relay accurate and relevant information, and keep family updated on your status. We stay by your side until you are admitted or discharged back home.

Hospitalizations
If you are hospitalized, we visit every day, doing our best to meet the rounding physician and the floor nurse to get first hand updates and information on your status. We speak with the hospital’s case manager to guide choices and referrals made in discharge planning.

Care Transitions
No matter where you go, we will follow to create a smooth transition. We monitor therapy progress at rehab and ensure you're getting what you need. For discharge home, we will make sure all supplies are obtained and support services are initiated.
