Assessment & Care Plan
Whatever situation you are facing, we will identify specific challenges and create a care plan full of strategies and solutions. We delineate care options so you better understand where you are in the process and what choices are available to you. We help you make educated decisions in elder care.
What’s the right answer?
When embarking on a caregiving or care decision journey, you need a road map.
Who is/are the decision makers? What if we don’t agree on what to do?
What type of care and how much is needed?
Can we stay home? How can we make the home safe?
Is it time for a facility? Which one? What’s the difference?
What can be done medically? What solutions are there besides medications?
How much will it all cost? What does insurance cover? How can we afford it?
Let our team take the guesswork out of your journey and give you clear answers on the best steps to move forward.
Trust the Experts
When something is important and you want to get it right, you hire a professional. Few things will impact an aging person’s health and wealth more than their care choices. Our team members have experience in the aging care industry and are familiar with local partners. We are members of the Aging Life Care Association and are held to their Standards of Practice and Code of Ethics. We are knowledgeable about an array of elder care issues and will conduct an assessment that covers:
Physical health & abilities
Medications & health history
Social/Emotional health & preferences
Cognitive health
Home safety
Legal preparedness
Finances, Insurance, & Benefits
The results of our assessment are compiled into a report with clearly identified areas of concern and strategies to resolve or improve the situation. We present this care plan to the family and clients to explain the strategies and work through any concerns. When desired, we will implement the care plan strategies directly, with family involvement.
